CONSORT, which stands for Consolidated Standards of Reporting Trials, encompasses various initiatives developed by the CONSORT Group to alleviate the problems arising from inadequate reporting of randomized controlled trials (RCTs).

The main product of CONSORT is the CONSORT Statement, which is an evidence-based, minimum set of recommendations for reporting RCTs. It offers a standard way for authors to prepare reports of trial findings, facilitating their complete and transparent reporting, and aiding their critical appraisal and interpretation.

The CONSORT Statement comprises a 25-item checklist and a flow diagram, along with some brief descriptive text. The checklist items focus on reporting how the trial was designed, analyzed, and interpreted; the flow diagram displays the progress of all participants through the trial.

Considered an evolving document, the CONSORT Statement is subject to periodic changes as new evidence emerges. This website contains the current definitive version of the CONSORT Statement and up-to-date information on extensions.

Professor Archie Cochrane (1908-1988)[1] was born in Kirklands, Galashiels, Scotland. He qualified in 1938 at University College Hospital, London, at University College London and joined the Medical Research Council's Pneumoconiosis Unit at Llandough Hospital, a part of Cardiff University School of Medicine in 1948. Here he began a series of studies on the health of the population of Rhondda Fach ...— studies which pioneered the use of randomised controlled trials (RCTs).

Archie Cochrane’s experiences during the Spanish Civil War, where he served as a member of a British Ambulance Unit, and later during World War II as Medical Officer at a number of prisoner of war[2] camps, had a profound and lasting effect on his future practice of medicine.

In 1960 he was appointed David Davies Professor of Tuberculosis and Chest Diseases at the Welsh National School of Medicine, now Cardiff University School of Medicine and nine years later became Director of the new Medical Research Council’s Epidemiology Research Unit at 4 Richmond Road, Cardiff.

His 1971 Rock Carling monograph Effectiveness and Efficiency: Random Reflections of Health Services[3] was very influential. These ideas and his advocacy of randomized controlled trials eventually led to the development of the Cochrane Library database of systematic reviews, the establishment of the UK Cochrane Centre in Oxford and the international Cochrane Collaboration.

The Cochrane Archive [4] is held at Llandough Hospital, Penarth.

Sepsis accounts for 8% of annual United States deaths, with approximately 200,000 sepsis deaths annually. The “Surviving Sepsis Campaign: International Guidelines for Management of Severe Sepsis and Septic Shock: 2008” (SSC) was published in January 2008. Unfortunately, significant controversy exists regarding these new guidelines.
Studies of implementation have revealed that hospitals are not fully implementing the guidelines. However, studies based on the implementation of the guidelines have demonstrated decreased mortality rates. The use of corticosteroids in septic shock is a controversial subject, with many trials using high-dose corticosteroids failing to demonstrate an improvement of survival rates. However, studies of the use of low-dose corticosteroids have given mixed results. The identification of pathogens must be achieved as quickly and accurately as possible, as treatment delays lead to increases in mortality, morbidity, and cost. Relatively new areas of investigation for the treatment of sepsis include the use of angiotensin-converting enzyme (ACE) inhibitors, such as ramipril, candesartan, captopril, and enalapril, and macrolides, such as azithromycin, roxithromycin, clarithromycin, and telethromycin.
